Your healthcare provider may prescribe or order in-home care when you’re recovering from a surgery or procedure, managing a chronic condition or have a combination of healthcare needs. Nursing care, vital signs and blood tests, physical therapy and other healthcare services can be brought to you so that you can recover sooner and in the comfort of your own home.

Kaiser Permanente Members
Hospice care is provided by a team of healthcare professionals and trained volunteers. Care is focused on honoring patients’ wishes and ensuring their personal dignity. Hospice care is provided where the patient lives, whether at home or in a care facility.
The Hayward Hospice Program provides services to Kaiser Permanente members who reside in the Greater Southern Alameda Area, including the cities of San Leandro, San Lorenzo, Castro Valley, Hayward, Union City, Newark, and Fremont.

Palliative Care
Palliative care is a specialized medical approach focused on providing relief from the symptoms, pain and stress associated with serious illness. A team of healthcare professionals works together to address the physical, emotional, and psychosocial needs of patients and their families as they face cancer, heart disease, neurological disorders, or other serious illnesses.
Palliative care is not limited to end-of-life care, as it can be initiated at any stage of illness and is often administered alongside curative treatments.
